  3. Fill out the Non-Employee NetID Creation Request form completely, beginning with the non-employee's personal information. Required fields are noted with an asterisk and listed below.

    1. Legal First Name & Last Name


    2. Gender (Female/Male)
    3. Date of Birthday (mm/dd/yyyy)


    4. Home City, State, and Zip Code


    5. Primary Phone
    6. Primary Email
    7. End Date (Max one (1) year)
    8. Work Location (Building/Room/Extension)


    9. Relationship to the University

  4. Check the boxes for resources(s) being requested for the applicant(s). Please call the Help Desk if you require clarification.

  5. Press the Submit button when the form is completed.6.  You will get the following message when your request has been successfully submitted.
